For what its worth, I have migrated the "Support for DITA 1.1" pages that
we discussed to the DITA OT Wiki, assimilated under this page:
http://dita-ot.wiki.sourceforge.net/Transfer+from+DFA--Support+for+DITA+1.1

Also I deleted these original pages at DITA Focus Area. I'll develop a
backfill topic for the known gap of knowledge about bookmaps and bookmeta.
