what is the intended encoding for a namespace declaration in a dtd.
is the "declaration before use" rule intended to apply here?
will the norm be that at least the xmlns:? attribute declaration always
appears before the respective element declaration?
will one attribute be expected per element (whether as a norm in the internal
subset, or not) in order to declare a default uri and bind the prefix?
will the choice of the elements which are afforded xmlns attributes in effect
specify the 'relative' root elements?
