Thanks to Andrew for the answer to my question. Unfortunately, it raises a couple of other questions:
1. I assume reserved attributes such as "xmlns" need not be declared in DTD's (or other schema mechanisms). Is this correct?
2. Is there, or will there be, a centralized definition of XML reserved objects (attributes, processing instructions, etc.) that may be defined outside the XML spec, but which cut across general usage of the language? I think there should be.

The processing instruction was used in drafts of the Namespaces
specification until approximately August 1998, at which time it was

I have seen several references to the processing instruction
"<?xml:namespace". I have also read the "Namespaces in XML" spec, which
discusses the "xmlns:" attribute. The relationship between the two is not
clear to me. My confusion may be connected to the fact that I have been
unable to find any formal documentation of "<?xml:namspace" or other
processing instructions. Is there any such formal documentation?
