I am a bit of a newbie myself but I have been lurking. Your inability to find the MSXSL application is because it was a early Microsoft implementation demonstrating the use of XSL. Some time afterwards the XSL draft changed and the application was removed from the Microsoft site. As XSL is still evolving your book probably deals with an earlier implementation.
