The URL has changed. The overall article is at:

And the actual survey is at:

Although the survey does appear to be aimed at whether software patents
should be granted in Britain, it is also being used as input to whether
they should be allowed in the EU. The deadline for EU input is 15
December, so getting input today or tomorrow would be a good thing.
There is no deadline for input on the British survey.
