In terms of categorisation of error or response types, the 'common' business response structure (and indeed the one that we currently return inside the SOAP Fault details section) does contain both a specific code (with an agreed semantic) AND an classification. For the most part callers appear to be mostly interested in the lower level codes but the classification does help us organise these into semantic groups.
 
I guess the main point of my note above is in some ways related to syntax (thats probably why I'm slightly uncomfortable). That is, a caller would to understand (parse and extract relevent information) from a number of potentially different message structures rather than the service just returning a common one that still carries the same basic information.
 
You have given me an idea though. I will take another look at the SOAP 1.2 spec iro errors (we are still using SOAP 1.1, but that doesn't mean we can't use the ideas of 1.2 in a 1.1 compliant way).
