The answer is of sufficient magnitude to fill a book. May I recommend the excellent "Data and Reality" (Kent, 1978, http://www.amazon.com/Data-Reality-William-Kent/dp/1585009709).

What do humans have to do with any of this?

I think you and Simon are really in violent agreement.  "Context" is ultimately a construction of humanity. I think, for  example, RDF folks were slow to understand this.
