On choosing whether to place a co-occurrence constraint on the
descendant or ancestor, it may help to consider the most desirable
behaviour if an author is crafting an XML instance by hand using an
XML Schema 1.1 validating editor.

One suggested approach that an editor tool might use when adding a
descendant element value, where the ancestor element is already in
place:
If the constraint's on the ancestor, then, when an author places the
editor cursor within a constrained descendant element to enter/modify
a value, the auto-complete list for permitted values would be filtered
to take the constraint into account. However, if the constraint is on
the descendant, then the auto-complete list would list all possible
values; this would be regardless of the ancestor context, but might
highlight those values that, if selected, would require a change to an
ancestor to make the document valid.

Then there's the need to consider the editor behaviour when an author
wraps an existing element or set of elements with an ancestor element
where a co-occurrence constraint exists. I have the feeling that
providing helpful behaviour in XML editor tools (such as auto-complete
lists) is going to be an interesting challenge with XML Schema 1.1...
