It would be nice if it were nicely layered like that, but especially 
when it comes to parameter entities, it isn't. It's all horribly 
recursive. For example, you can't just expand &foo; to assemble a 
character stream, and then parse the stream -- elements have to be 
properly nested within entity boundaries, so the layers are deeply 
entangled.
