Surely it means whatever you specify it to mean.

Reasonable specifications might be any of


empty content means the name is unknown (ie like the meaning that you assigned to your first case)

empty content is same as omitting the element and means there is no middle name (ie the meaning you assigned to the third case)

empty content is invalid so the document is invalid and means nothing.
