Incorporation of Resolution to BP-28 - New WS-HT Baseline: CD-04Rev 2

I've merged into the HT spec the latest version of the composite task / sub-task / routing pattern proposal edited during the F2F and subsequently posted by Ravi Rangaswamy at http://www.oasis-open.org/apps/org/workgroup/bpel4people/email/archives/200906/msg00030.html. 

The updates have been committed to SVN as revision 134. This version of the spec (CD-04 Rev2) now forms the baseline for future work.
