[chairs] Revised filenaming proposal

I have attached the revised filenaming proposal.  Karl and Eduardo, you 
should consider this to be "submitted by the chairs" to you for 
consideration at this point.  Also, Karl, can you tell us the 
disposition of the unique numbering plan?  Are you prepared to do that soon?
