Annex A Content and front-matter meta data

Reviewing the meeting attendance and voting status, I believe that
the following would be appropriate.  Note that individual and
associate members cannot have their affiliation listed, so I've
guessed from the clues (as indeterminate as they are) in the roster
as to who are not organizational members.
