Latest CVA public review draft edited under OASIS supervision

With many thanks to Mary for her guidance, here is the latest set of 
documents for the public review draft for context/value association files:

   (a history of all revisions is found through that URI)

Today an annex was added that simply points to the namespace URI as 
the location where users find the CVA schema fragments.
