Formal request for leave of absence until July 1, 2023

(I apologize for my cut/paste laziness in the earlier post as I am
trying to get many things accomplished as quickly as I can; thank you
for your patience.)
Fellow Code List Representation TC members,
Per procedure, this is my posted formal request for a leave of
absence starting Friday March 3 until July 1. I'm requesting an
exception, please, to the stated limitations due to this being a
health-related leave.
