Proposed changes to LwDITA

Dear LwDITA subcommittee,
I am working on the agenda for our call next week, but here's a file for discussion that I wanted to share with you as early as possible.  I have condensed all the proposed changes to LwDITA that came from alignment with IBM marked-it, with Jarno's Markdown DITA syntax, and with proposed/approved changes to DITA 2.0 that affect LwDITA.  Please take a look before our meeting and come prepared to discuss them.
