Issue: glossary.dtd does not conform to 1.2 spec filenaming rules

Actually, I'd much rather have the module named "glossary" rather than
"glossentry." I think "glossentry" is confusing for everybody. I never
remember that that is where I'm supposed to start. In general, the whole
thing needs to be fixed for 1.3
