Proposal 13111: MathML Updated For Equation Numbering

I have posted updated versions of the 13111 State 3 proposal. This changes
the specialization base of <equation-block> from <p> to <div> and adds the
new <equation-number> element. I've updated the declarations to reflect
this change. I've tested the updated declarations in the context of the
general RNG-to-DTD/XSD/RNC process although I have not yet committed those
changes to the SVN repository.
