Stage 2 Proposal #34: Remove topicset and topicsetref Elements

I have prepared the first draft of the Stage 2 proposal:

https://www.oasis-open.org/apps/org/workgroup/dita/download.php/62676/proposal-34-stage-2.html

I have made all the required grammar and specification changes and created a pull request to the TC's git repository:

https://github.com/oasis-tcs/dita/pull/103

Cheers,

E.
