it's too bad that I have to set this twice - in xml sources and xsl
customizations...

<book id="impnotes-top"> is the root element, and if I do not supply an
id, the generated html will contain a gensym id.

the top-level toc file is named "impnotes-top.html" and I have to
symlink it to "index.html".
