multiple mathphrase db 5 equation w/ db-xsl-1 generates phrases without line breaks between them

I have a test case where a db5 equation with multiple <mathphrase>
elements has no line breaks between the individual phrases as it does
in the case of <mediaobject> and <mml:*>. This is common across xhtml,
html, and pdf output.
