Can you maybe give me an idea how to insert the word Chapter followed by the current chapter number to the header? 

By inserting <fo:retrieve-marker retrieveclassname="section.head.marker">, I now have the name of the chapter in the header. In addition to this, I would like to have the word Chapter + the current chapter number. For example: Chapter 1. Introduction.

I don't think you are doing anything wrong. But the page number prefix
customization was made for a rather old version of DocBook XSL. I guess that
modifications in later versions clash with this customization. It's not much
of an answer, but I haven't looked very deeply into this.
