It seems fox:destination was not supported in FOP 
0.93, but it is in later versions:

FOP 0.93 is four years old now, so you might 
consider updating.

I'm using DocBook XML v. 4.5, docbook-xsl.1.76.1, and FOP 
  0.93 and got the following error: "No element mapping definition found for

Not sure why I'm getting this error since I've set the
