DocBook 4.0b5

Folks,

I uploaded b5 yesterday. Nothing really changed, I just added a
note to the 40chg.txt file (how did we forget to mention that
we'd renamed DocInfo to *Info?) and removed 'beginpage' from
highlights.mix in the XML version. (It should never have been
there, it's prevented in the SGML version by exclusion.)
