more documentation fixes

Hi Norm,
I'm looking at the Definitive Guide pages for <table> and it doesn't
describe the HTML table model.  The content model was updated, but there is
no description of it, or cautions about mixing models.

Also, the caption element doc needs to be updated to mention its use in the
HTML table model.
