I'm amazed at how well you managed to capture this.  One nit about:

Larry: Already using existing elements for modules, and
developed a layer to specify hierarchy and relationships.

It is actually a separate schema, similar to what Jirka came up with.
It is not part of our customization of the DocBook DTD, so I don't think
of it as a layer, but rather as a supplemental file.  Perhaps something
like:

developed a supplemental markup in a separate file to specify
hierarchy and relationships among modules.
