Transclusions to XInclude mapping

Hi folks,

please find attached updated version of transclusion proposal, which now
contains new appendix C with mapping from transclusions to XInclude.

My first impression is that XInclude and state of its implementations
are not powerful enough to be used as a replacement for transclusions.
