DocBook CSD02/CSPRD02 drafts

Hi Chet,

This morning, I uploaded the 02 drafts of the DocBook CSD and CSDPRD.
These versions include schema and documentation fixes for all the
comments received in the 01 period (plus fixes for a couple of last
minute technical errors reported about a week ago).

I added a "disposition of comments" section to Appendix B.

Please let me know what steps if any I need to take to push this ever
