DOCBOOK: DocBook Stylesheet ?

Question to all (and to Norman in particular)

What stylesheet is used to produce the print version of "DocBook - The
Definitive Guide" ? Rendering a document using the DocBook modular
stylesheets does not seem to produce the same output. In particular, the
page headers/ footers are not the same: the page number is on the header and
the header is separated from the text by an horizontal line. The footer is
empty.
Is this the result of the use of another stylesheet (if yes where can I find
it ?) or do I miss something ?
