<emphasis> within <indexterm>?

All,

I frequently run into the need to place either the <emphasis> tag or the <citetitle> tag into an index entry whenever the title of a publication is referenced. Has anyone else encountered these situations and, if so, should this be considered for a change for the next version of the DTD?
