Link to RFE tracker, official DocBook

It seems like it'd be useful to have a link to the Sourceforge RFE
tracker somewhere on the http://www.oasis-open.org/docbook/ page
and at http://www.oasis-open.org/committees/tc_home.php?wg_abbrev=docbook

And it looks like we no longer have a statement anywhere clearly
saying what the current committee-approved version of DocBook is;
someone taking a quick look at http://www.oasis-open.org/docbook/xml/
might think it was actually v5.0
