One added point in the first half.  The section 4.2.1 is 
missing -- we skip from 4.2 Error Handling Module to 4.2.1.1 Definitions.  
We should at least have an intermediate heading or (probably easier) make 
4.2.1.1 and 4.2.1.2 be 4.2.1 and 4.2.2.
