[ebxml-msg] Issue 42: Acknowledgment/From

To be clear, my recommendation is removing this element from our specification
and schema.  It solves only a very small part of the tracking and tracing
issues inherent in hop-to-hop acknowledgements, is not completely described
and doesn't give the receiving MSH something to act upon.
