Issue 92, clock synchronization

Here is proposed text to close Issue #92, which is to provide a note 
about clock synchronization in section 3.1.6.4.  Insert as a new 
paragraph after line 900:

   It is STRONGLY RECOMMENDED that the system on which an MSH is deployed
   use a mechanism to synchronize its internal clock with a trusted,
   accurate source, in order to minimize differences between From Party
   and To Party clocks.
