Document Description:
Proposed updates on the messaging model. Added "contexts of use" for an
MSH. Refined terminology. Make it clearer that the MSH may be a SOAP
intermediary. Propose to integrate the MEP section in the Messaging Model
section. Some minor updates need be made to the MEP section, listed here.
