First draft attempting to pull together conversations on Streams and  Signals.

 Does not yet incorporate restriction elements, smoothing, and the special  case of the EventInterval.

 Second Draft:
 Includes reference to CS01 WS-Calednar semantics, fuller treatment,  responses to many comments.

 For incorporation into current Chapter 4 to avoid multiple re-defintions  later.

 Added distinctions for Observational Streams.
