Use case template

Following our meeting earlier in the week, I attach:

* Use case template d0.01.rtf
* Notes on use case template d0.01.rtf

I will send an example use case shortly to assist in the application and
understanding of the template.

Jason has reviewed the draft and helped to knock off some rough edges and
make other improvements. Please let me have any suggestions for revision
before we all put it to work.
