Attached are results of adding in lists.  The test matrix is growing a good bit larger, so I made this document significantly more terse.  One table shows the reduced matrix of behavior and the other shows visuals of each test case, results only for brevity.

I noticed a formatting error while working on the paragraph merging with lists cases.  On page 4 of the previously attached PDF, in the last example on the page, the result shown for removing the empty plain-formatted line below the heading-formatted one is incorrect.  The word "Heading" should obviously still be heading-formatted.  Apologies for missing that when I reviewed the document.
