Michael,
The terms "report or form" may be meaningful to you but as Dennis points out, they are not defined in this context.
The content of the <db:component> element currently reads:
"A database component is either referenced using the xlink:href attribute or it is contained in a child <office:document> or <math:math> element."
Which of those goes with "report" and which goes with "form" I cannot say.
If there is some definition of "report" and "form" that I have overlooked that should be referenced here please point it out.

PS: After thinking about it you must mean that <office:document> contains a form or report but I don't know what <math:math> would contain.
