First pass at C++ header files

I've added a frst stab at headers to
the svn repository: http://tools.oasis-open.org/version-control/browse/wsvn/sca-c-cpp/SCA_CPP_HEADERS/

This is work in progress and there is
more to come.

The copyright statment I borrowed form
the Java interface files but have updated the end-date to 2009.

Do we want to include Javadoc (Doxygen)
style comments? Pro: nicely documents the methods. Con: spec is the definitive
source so can get out of step. I think we should add the spec words as
comments.
