Core-12 with renaming of elements

All,

	Since some folk seem to have difficulty lookng at the raw schema to
see the impact of the schema renaming, here is core-12 with the renaming and
change bars.

	I have not added in the substitution groups or performed any of the
major structural re-orgs. However those will only impact the schema
fragments and not the text to any great degree.
