SOAP Header

Suppose I wish to add SAML assertions to a soap message as it passes 
through my processor.  What's the common method to do that -- just put 
the saml:Assertion element as the SOAP header itself:
  SOAP:Envelope
    SOAP:Header
     SAML:Assertion ....
Or are folks already wrapping inside some ws-security element or 
something else?
