proposed resolutions for issues 13, 15,

Issue 13: remove the extra completionStatus operation
Issue 15: CompleteWithStatus should send a CompletionStatus not a Status as
currently in
the WS-Context schema.
Issue 17: change all occurrences of wsc4c in all schemas to wsctx.
Issue 18: fix the import statement.
Issue 19: add the part attribute where necessary.
Issue 20: check that this isn't fixed by Issue 13, and if it is not, then
ensure all declared operations are in the PortType.
