qandaset/qandadiv (RFE #1194196)

In RFE 1194196, Orhan asks for the ability to mix qandaentry and
qandadiv arbitrarily in a qandaset.

Last month the TC discussed this at some length without reaching
consensus. I think it's fair to say that there's a general sense that
allowing qandaentry to occur between qandadivs would be a significant
change that's not in the same spirit as other DocBook content models.

The example provided in the RFE is complete, but it's not a real
example. Orhan, can you provide a real world example of where and why
you want to mix divs and entrys?
