fixed 2 issues with the schema proposal:

1. the text:sender-initials field was reused by reference, but this new element is not a text field and in particular the text:fixed attribute makes no sense for it because it is always fixed.
2. the new element was inserted at the wrong position into the parent - metadata elements must be *before* the text:p / list children that make up the content of the annotation (LibreOffice would not import the element if it comes after the content).
