Another way of looking at this:  "Some People" have very strong
assumptions about the scale of documents XML should work with.

Now, we all know that scaling assumptions like that are dangerous.
(Or if we don't, it's time to learn it!)

A better approach is to recognize the document scaling is one
of the issues that designers must deal with, external entities
are one of the tools available to manage it, and that whenever
you have lots of data to deal with you're going to have to be
very careful.
