Theres a lot of DTD stuff off  alphaWorks, a free or trial based IBM site.
They may not have exactly what your looking fo but they have a Lightweigth
Extractor and a Translator Generator.

What tools can people recommend for creating DTD's (+ XML schemas?)
and editing XML documents?

The tools should be quite visual (e.g. a graphical representation of
the structure of a DTD/XML document). The ability to validate an XML
document while editing would be good. Cross platform tools
(specifically, available for Windows and Mac OS) would be a benefit.
