Found it.  It wants the prefix on the XPath 
expressions all the way down the path.

<xsl:copy-of select="/cad:ips_cad/cad:event/cad:eventID"/>

Mike Kay's note in his excellent book, 

"within an XPath expression, the default is never used for an unprefixed
name"

makes sense. Sorry for the noise.
