I have written several models for various clients.  Although the
question/answer model seems relatively simple it can get quite complicated
and it can have many levels of hierarchy.  Although, I would recommend it
for most applications, the original DoD IETM (Interactive Electronic
Technical Manual) specification is typically a question and answer model
when you get into fault isolation.

You also need to look at the context what you want to model question and 
answer. A textbook question and answer model would be different than
testimony which consists of questions and answers.  One of the models
I wrote was for congressional hearings.  In this case we had to take
into account things like 'laughter', disruptions from the floor, or
any other anomaly that could occur.

You may want to look at what the IMS Global Learning Consortium 
(http://www.imsglobal.org) Question and Testing Specification if
you are looking for a traditional learning module.
