some ideas :
due to XML itself :
-the physical structure of some XML constructs : notations, entities,
cdata sections, and of course DTDs... DTDs are widely used, is that
implies that most documents are smelling ?
-default attribute values
ambiguous documents :
-elements without namespace URIs
data model :
-parsing with a non-validating parser
-parsing with no support of namespace URIs
misc:
-using processing instructions
-using things that could be externalized from xml instances : i'm
thinking about <!DOCTYPE> or xsi:schemaLocation ; as they apply to a
document class, they have nothing to do inside an instance
